At Rockcliff Oral Surgery, it is our mission to provide the highest level of personalized health care, in a comfortable and caring atmosphere.
In this office we specialize in surgery of the mouth and face including removal of troublesome third molars (wisdom teeth). It is this kind of problem that will most likely bring you to us for what may well be your first exposure to surgery. Even though our encounter is usually brief, we try hard to help you understand the nature of this surgery and the reasons for it and any associated risks.
You will be sent a questionnaire to fill out and return to the office before your appointment date. It is essential that you take a moment to complete this form and return it within 48 hours. A self-addressed stamped is provided for your convenience. This will minimize your waiting time at your first appointment.

YOUR FIRST VISIT – WHAT TO BRING
You are responsible for checking with your employer to determine if your surgery will fall under your medical or dental coverage. Please complete all of the insurance information section on the enclosed form. This will insure maximum benefits will be paid by your insurance company. Be sure to bring your insurance card and a completed form on the day of your appointment. We participate with Blue Cross Blue Shield of NC, Cigna, Delta Dental, Metlife, Assurant and HealthScope.
Teeth displaced within the jaw are difficult to examine with conventional dental x-rays; therefore, we use a panoramic radiograph (panorex) to insure the proper diagnosis and preoperative evaluation. This radiograph will provide an overall view of both the upper and lower jaws and the relationship of the teeth to the adjacent nerve and blood vessel canals, as well as to the nasal and sinus cavities. We do accept panorex x-rays taken by your general dentist if they are of acceptable quality and have been taken within a year of your surgery. If this is not provided on the day of your consultation, the necessary x-rays will be taken at an additional charge.
EXAM AND CONSULTATION:
The information you have provided will aid our doctors in providing you with the best course of treatment based on your individual medical needs. After examining your x-rays and medical history, they will discuss with you the procedure they will be performing, explain any risks or possible complications and answer any questions you may have. It is our intention to help you understand your anticipated surgery and thereby alleviate any preoperative anxiety you might have.
Bring a list of all of your current medications and the daily dosages. Patients under the age of 18 will require a parent or legal guardian to accompany them to their consultation appointment, At that time, the surgical consent form and insurance assignment will need to be signed and surgical fees discussed.
We recognize the attention you must give to the costs of your health care. If you have not already received one, please ask for our financial policy brochure to answer any questions you may have.
KEEP IN MIND:
Scheduled appointments must be confirmed 48 hours before your appointment time. Please call our office at 255-7781 at least two business days prior to your scheduled visit and ask to speak to your doctor’s appointment secretary. Your health and well-being are our primary concern. We and our staff will be happy to discuss any questions or concerns you may have concerning your anticipated treatment.
